Build Independence Where It Matters Most — At Home
@Home Therapy is seeking a licensed Occupational Therapist to provide in-home services to adult and geriatric patients in the Great Neck area. This per diem opportunity is ideal for clinicians who value autonomy, flexible scheduling, and meaningful one-on-one care in a home setting.
In this role, you’ll focus on helping individuals safely navigate daily routines, regain functional abilities, and remain independent in their own environment.
What You’ll Be Doing:
Functional Therapy in the Home:
- Assess patients’ ability to perform everyday tasks such as dressing, bathing, and meal preparation.
- Identify environmental barriers and recommend modifications to improve safety and accessibility.
Individualized Treatment Approach:
- Create targeted therapy plans that support independence and long-term function.
- Incorporate adaptive techniques, equipment training, and energy conservation strategies.
Ongoing Patient Support:
- Monitor progress and adjust interventions based on patient needs and goals.
- Provide hands-on guidance to caregivers to reinforce therapy outcomes.
Documentation & Coordination:
- Complete visit notes using a web-based EMR system.
- Collaborate with interdisciplinary team members when needed.
Why Clinicians Choose @Home Therapy:
- Flexible Caseload Options: Design a schedule that fits your availability — take on as many or as few patients as you prefer.
- Efficient Back-End Support: Administrative staff assist with coordination and logistics, allowing you to stay focused on clinical care.
- Competitive Per-Visit Compensation: Transparent pay structure with opportunities to increase earnings based on your caseload.
- Clinical Growth Opportunities: Access optional resources and mentorship to expand your skills in home-based care.
Qualifications:
- Active New York State Occupational Therapist license.
- Interest in adult and geriatric populations.
- Strong interpersonal and patient education skills.
- Comfortable working independently in a home care environment.
- Familiarity with EMR systems (or willingness to learn).
Who This Role Is Great For:
This position is well-suited for OTs who prefer:
- A less structured, field-based workday.
- Building direct relationships with patients in their homes.
