Help Patients Stay Independent in Their Own Homes
@Home Therapy is hiring an Occupational Therapist to serve adult patients across Brooklyn. This role centers on delivering practical, home-based interventions that help individuals maintain independence, improve safety, and function more confidently in their daily routines.
We’re looking for clinicians who enjoy working outside of a clinic setting and value building meaningful connections with patients in their own environments.
What This Role Involves:
Home Mobility Assessment:
- Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
- Identify environmental and physical barriers that may affect safe mobility and independence.
Functional Rehabilitation Services:
- Develop and implement individualized treatment plans to improve or maintain mobility, strength, balance, and functional independence.
- Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.
Real-World Movement Training:
- Train patients in safe transfers, ambulation, stair negotiation (when applicable), and household mobility.
- Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.
Ongoing Progress Monitoring:
- Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
- Collaborate with physicians and the interdisciplinary care team to support patient goals.
Documentation & Compliance:
- Complete timely documentation using the agency's electronic medical record (EMR) system.
- Maintain compliance with clinical, regulatory, and agency documentation standards.
Work Style & Flexibility:
- Set your own schedule and service area within Brooklyn.
- Independent contractor role with autonomy over workload.
- Minimal administrative burden with support from coordination staff.
Qualifications:
- Current New York State OT license.
- Interest in working with adult and geriatric populations.
- Ability to work independently in a field-based setting.
- Strong communication and problem-solving skills.
- Basic comfort with digital documentation systems.
