Deliver Real-World Therapy in a Dynamic Community Setting
@Home Therapy is seeking an Occupational Therapist to provide in-home services to adult patients across Brooklyn. This role focuses on improving functional independence by addressing how patients manage daily activities within the context of their home and neighborhood environment.
You’ll work directly in the community, helping individuals build practical skills that support safer and more independent living.
What You’ll Be Doing:
Functional Assessment in Real Environments:
- Evaluate a patient’s ability to perform daily activities within their home setting.
- Identify barriers related to layout, accessibility, and routine task performance.
Skill-Building for Daily Life:
- Work on self-care tasks such as dressing, grooming, and meal preparation.
- Support patients in developing strategies to manage household routines more effectively.
Adaptive Strategies & Recommendations:
- Introduce compensatory techniques to improve independence and reduce strain.
- Recommend environmental adjustments to enhance safety and usability.
Caregiver Collaboration:
- Educate family members or aides on safe assistance techniques.
- Reinforce therapy goals between visits for consistency.
Clinical Documentation:
- Maintain accurate visit records using an electronic EMR system.
- Ensure documentation aligns with regulatory and clinical standards.

Qualifications:
- Active New York State Occupational Therapist license.
- Experience with adult or geriatric populations preferred.
