Deliver Functional Care in a High-Need Community Setting
@Home Therapy is seeking an Occupational Therapist to provide in-home services to adult patients in the Elmhurst area of Queens. This role focuses on supporting individuals living in dense urban housing environments where daily functional challenges often require practical, adaptive solutions.
You’ll work with a diverse patient population while helping improve independence in real-world home settings.
Compensation & Incentives:
- $500 bonus after 50 completed visits.
- Additional earning opportunities available based on caseload volume.
What You’ll Be Doing:
Functional Home Assessments:
- Evaluate patients’ ability to safely complete daily activities in apartment and multi-unit housing settings.
- Identify barriers related to space limitations, mobility, and accessibility.
Practical Skill Development:
- Support patients with daily living tasks such as dressing, bathing, and meal preparation.
- Focus on improving efficiency and safety within compact living environments.
Adaptive Intervention Strategies:
- Introduce compensatory techniques tailored to urban home layouts.
- Recommend tools and strategies to improve independence in constrained spaces.
Caregiver & Support Education:
- Provide instruction to caregivers or family members on safe assistance techniques
- Reinforce therapy strategies between visits.
Clinical Documentation:
- Complete visit notes using a web-based EMR system.
- Maintain compliance with regulatory and clinical documentation standards.
Work Structure & Environment:
- Flexible Scheduling: Set your own availability and manage your caseload independently.
- Urban Community Caseload: Provide care across Elmhurst and surrounding Queens neighborhoods with high patient demand.
- Operational Support: Administrative coordination and scheduling assistance provided.
- Scalable Caseload: Opportunity to increase visit volume based on availability.
Qualifications:
- Active New York State Occupational Therapist license.
- Experience with adult or geriatric populations preferred.
- Strong ability to adapt therapy to varied home environments.
- Excellent communication and problem-solving skills.
- Comfortable working independently in a field-based role.
