About the Job
@Home Therapy is looking for a compassionate and motivated New York State licensed Occupational Therapist to provide one-on-one care to adult and geriatric patients in their homes.
This opportunity is ideal for an OT who enjoys independence, flexibility, and making a meaningful difference in patients' daily lives.
Responsibilities Include:
- Completing in-home OT evaluations and treatment.
- Developing individualized treatment plans.
- Addressing daily living skills, mobility, coordination, cognition, and home safety.
- Educating patients and caregivers on adaptive techniques and equipment.
- Monitoring progress and updating care plans.
- Completing EMR documentation and collaborating with the care team.

Qualifications:
- Active New York State OT license.
