Deliver Practical, Real-Life Therapy in Patients’ Homes
@Home Therapy is seeking a licensed Occupational Therapist to provide in-home services across New York City. This role focuses on helping adult patients improve their ability to safely and independently manage daily activities within their own living environments.
You’ll have the opportunity to evaluate, treat, and guide patients using personalized strategies that directly impact their day-to-day function.
What You’ll Be Doing:
Functional Evaluations:
- Assess patients’ ability to perform daily activities within their home setting.
- Identify physical, cognitive, and environmental barriers to independence.
Treatment Plan Development:
- Design individualized therapy programs tailored to each patient’s needs.
- Incorporate adaptive techniques, equipment recommendations, and safety strategies.
Hands-On Therapy:
- Work with patients on tasks such as self-care, home management, and mobility within the home.
- Address coordination, upper extremity function, and activity tolerance.
Patient & Caregiver Guidance:
- Educate patients and families on techniques to improve independence and safety.
- Provide recommendations for home modifications when appropriate.
Documentation & Compliance:
- Complete timely documentation using an electronic system.
- Ensure all services meet regulatory and clinical standards.

Qualifications:
- Active New York State Occupational Therapist license.
- Experience with adult or geriatric populations preferred.
- Ability to work independently in a home care environment .
- Strong clinical reasoning and communication skills.
- Comfort using EMR systems.
