@Home Therapy is seeking a Speech-Language Pathologist to provide mobile, in-home services throughout Jamaica, Queens. This role focuses on evaluating and treating adults with speech, cognitive, and swallowing disorders in a real-world setting that allows for more meaningful, functional outcomes.
You’ll have the flexibility to structure your own schedule while delivering highly individualized care.
Core Responsibilities:
Clinical Evaluations:
- Perform comprehensive assessments of speech, language, cognition, and swallowing function.
- Identify deficits related to neurological, post-acute, or chronic conditions.
Targeted Treatment Interventions:
- Implement therapy techniques to improve communication effectiveness.
- Address cognitive-linguistic challenges such as memory, sequencing, and executive function.
Dysphagia Management:
- Evaluate swallowing safety and recommend diet modifications when appropriate.
- Train patients and caregivers on safe feeding techniques.
Home-Based Care Planning:
- Design treatment strategies that reflect each patient’s living environment.
- Adapt therapy methods to support real-life application and carryover.
Documentation & Compliance:
- Complete timely, accurate notes using an electronic documentation system.
- Follow all regulatory and agency standards for care delivery.

Qualifications:
- Active New York State SLP license
- Experience with adult or geriatric populations preferred.
