Support Functional Independence in Community-Based Home Care
@Home Therapy is seeking a Physical Therapist to provide in-home services to adult patients throughout North Hempstead. This role focuses on helping individuals manage chronic conditions, maintain functional ability, and improve safety in their daily home environment.
You’ll deliver hands-on care in a setting where therapy directly supports long-term independence and quality of life.
Core Responsibilities:
Functional Movement Evaluation:
- Assess balance, strength, endurance, and mobility within the home environment.
- Identify movement limitations affecting daily function and safety.
Ongoing Rehabilitation & Maintenance:
- Develop treatment plans focused on maintaining or improving functional capacity.
- Support patients with chronic conditions, age-related decline, or long-term impairments.
Home-Based Functional Training:
- Guide patients through real-world mobility tasks such as transfers and household navigation.
- Reinforce safe movement strategies to reduce fall risk and injury.
Patient Progress Management:
- Monitor changes in functional status over time.
- Adjust therapy plans based on patient response and goals.
Clinical Documentation:
- Complete visit notes using an electronic EMR system.
- Ensure compliance with clinical and regulatory requirements.

Qualifications:
- Active New York State Physical Therapist license.
