Provide Community-Based Rehabilitation in a Suburban Setting
@Home Therapy is seeking a Physical Therapist to deliver in-home services to adult patients in the Farmingdale area. This role focuses on helping individuals maintain mobility, manage physical limitations, and continue safe daily functioning within their home environment.
This position is well-suited for therapists who prefer consistent community-based work in a suburban caseload.
What You’ll Be Doing:
Home Mobility Assessment:
- Evaluate balance, gait, strength, and functional movement within the home.
- Identify environmental and physical barriers affecting safe mobility.
Functional Rehabilitation Services:
- Develop and implement treatment plans focused on maintaining or improving movement ability.
- Support patients with chronic conditions, age-related decline, or post-acute recovery needs.
Real-World Movement Training:
- Assist with transfers, walking safety, and household navigation.
- Reinforce safe movement strategies that reduce fall risk.
Ongoing Progress Monitoring:
- Track patient outcomes over time and adjust care plans as needed.
- Ensure goals remain aligned with functional independence.
Documentation & Compliance:
- Complete visit notes using an electronic EMR system.
- Maintain compliance with clinical and regulatory standards.
Work Structure & Environment:
- Flexible Scheduling: Choose your availability and manage your own caseload.
- Suburban Community Caseload: Provide care across Farmingdale and surrounding Long Island communities.
- Operational Support: Scheduling and administrative coordination handled by the office team.
- Scalable Workload: Ability to increase or adjust caseload based on preference.
Qualifications:
- Active New York State Physical Therapist license.
- Experience with adult or geriatric rehabilitation preferred.
- Strong understanding of functional mobility and balance training.
- Ability to work independently in a home care setting.
- Strong communication and clinical decision-making skills.
