Support Long-Term Mobility and Independence at Home
@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adult patients in the Jericho area. This role focuses on improving mobility, restoring functional movement, and helping patients maintain independence within their home environments.
You’ll be working in a community-based setting where therapy is directly applied to real-life movement challenges inside the home.

What You’ll Be Doing:
Mobility & Functional Assessment:
- Evaluate balance, gait, strength, and overall movement ability.
- Identify physical limitations impacting daily activities within the home.
Rehabilitation Programming:
- Develop individualized treatment plans focused on strength, endurance, and coordination.
- Support recovery following injury, surgery, or progressive conditions.
Functional Training in Real Settings:
- Work with patients on transfers, walking safety, and stair navigation.
- Reinforce movement strategies that improve independence and reduce fall risk.
Progress Monitoring:
- Track improvements in mobility and adjust treatment plans as needed.
- Set functional goals tied to daily living activities.
Documentation & Reporting:
- Complete visit notes using an electronic EMR system.
- Maintain compliance with clinical and regulatory standards.

Qualifications:
- Active New York State Physical Therapist license.
- Experience with adult rehabilitation preferred.
- Strong understanding of gait, balance, and functional movement training.
- Ability to work independently in a home care setting.
- Strong communication and clinical reasoning skills.
