What You’ll Do:
- Provide one-on-one physical therapy to adult patients in their homes.
- Perform evaluations, develop treatment plans, and deliver skilled interventions.
- Adjust care plans based on patient progress and functional goals.
- Coordinate directly with patients to manage visit scheduling.
- Complete documentation using our user-friendly EMR system.
- Maintain compliance with Medicare guidelines and company standards.
What We’re Looking For:
- Licensed PT in New York State.
- Experience with adult or geriatric populations (home care experience a plus, not required).
- Strong communication skills and ability to build patient rapport.
- Comfortable working independently in a home-based setting.
- Basic familiarity with EMR systems (training provided).
About the Role:
This is an in-home position, not a single-site clinic role. Patients are assigned based on your preferred geographic coverage area across NYC and Long Island.
