@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services across New York City. This position is centered on helping adult patients restore strength, improve mobility, and safely return to daily activities following injury, surgery, or chronic conditions.
You’ll work directly in patients’ homes, allowing you to deliver practical, outcome-driven care in the environments where it matters most.
What You’ll Be Responsible For:
Mobility & Strength Assessments:
- Evaluate gait, balance, range of motion, and functional mobility.
- Identify fall risks and physical limitations within the home.
Rehabilitation Programming:
- Design and implement exercise plans to improve strength, endurance, and coordination.
- Support recovery from orthopedic, neurological, or post-surgical conditions.
Functional Movement Training:
- Assist patients with transfers, ambulation, and stair navigation.
- Reinforce safe movement patterns to prevent re-injury.
Progress Monitoring:
- Track patient improvements and adjust treatment plans accordingly.
- Set measurable goals tied to functional independence.
Documentation & Compliance:
- Complete visit notes using a digital EMR system.
- Maintain compliance with healthcare regulations and agency standards.

Qualifications:
- Active New York State Physical Therapist license.
