@Home Therapy is looking for a Speech-Language Pathologist to provide in-home services to adult patients in the Hempstead area. This role focuses on helping individuals improve communication, cognitive function, and swallowing abilities—right where they feel most comfortable.

Your Impact in This Role:
Communication & Cognitive Therapy:
- Evaluate and treat speech, language, and cognitive-communication disorders.
- Support patients with memory, attention, and problem-solving challenges.
Swallowing & Safety Management:
- Assess and treat swallowing disorders (dysphagia).
- Provide recommendations to improve safe eating and reduce aspiration risk.
Personalized Care Delivery:
- Develop therapy plans tailored to each patient’s home environment and goals.
- Modify interventions based on progress and clinical findings.
Patient & Caregiver Education:
- Teach strategies that patients and caregivers can apply between visits.
- Promote long-term carryover of communication and swallowing techniques.
Documentation & Coordination:
- Complete visit notes using a digital EMR system.
- Communicate with care team members as needed to support patient outcomes.

Qualifications:
- Active New York State Speech-Language Pathologist license.
