@Home Therapy is seeking a full-time Care Coordinator to join our in-office team in Valley Stream. This role is central to supporting patients throughout their care journey—from initial referral through ongoing service coordination.

Role Overview:
As a Care Coordinator, you will manage patient intake, coordinate therapy services, and serve as a key point of contact between patients, therapists, and the administrative team. You’ll help ensure patients receive timely, high-quality care while keeping operations running smoothly.

Key Responsibilities:
  •  Coordinate new patient intake, including referrals, onboarding, and insurance verificatio
  •  Assign patients to appropriate therapists based on location and availabilit.
  •  Manage an active caseload and track visit completion and documentation.
  •  Communicate with therapists via phone and email to support scheduling and follow-ups.
  •  Conduct patient check-ins to ensure satisfaction and continuity of care.
  •  Maintain accurate patient records within the EMR system.
  •  Handle inbound calls and email inquiries from new and existing patients.
  •  Collaborate with supervisors and participate in regular team meetings.

Qualifications:
  •  At least 1 year of experience in a medical or administrative office setting.
  •  Strong communication skills (phone and email).
  •  Highly organized with strong attention to detail.
  •  Comfortable multitasking and prioritizing in a fast-paced environment.
  •  Proficient with computers, data entry, and Microsoft Office tools.
  •  Ability to present information clearly and professionally to patients.
  •  Team-oriented with a positive, proactive attitude.
  •  Reliable attendance and punctuality are essential.
  •  Experience with EMR systems is a plus.
