@Home Therapy is seeking a full-time Care Coordinator to join our in-office team in Valley Stream. This role is central to supporting patients throughout their care journey—from initial referral through ongoing service coordination.

Role Overview:

As a Care Coordinator, you will manage patient intake, coordinate therapy services, and serve as a key point of contact between patients, therapists, and the administrative team. You’ll help ensure patients receive timely, high-quality care while keeping operations running smoothly.

Key Responsibilities:

Coordinate new patient intake, including referrals, onboarding, and insurance verificatio
Assign patients to appropriate therapists based on location and availabilit.
Manage an active caseload and track visit completion and documentation.
Communicate with therapists via phone and email to support scheduling and follow-ups.
Conduct patient check-ins to ensure satisfaction and continuity of care.
Maintain accurate patient records within the EMR system.
Handle inbound calls and email inquiries from new and existing patients.
Collaborate with supervisors and participate in regular team meetings.

Qualifications:

At least 1 year of experience in a medical or administrative office setting.
Strong communication skills (phone and email).
Highly organized with strong attention to detail.
Comfortable multitasking and prioritizing in a fast-paced environment.
Proficient with computers, data entry, and Microsoft Office tools.
Ability to present information clearly and professionally to patients.
Team-oriented with a positive, proactive attitude.
Reliable attendance and punctuality are essential.
Experience with EMR systems is a plus.
